Incident Register - Location/Treatment

The Location/Treatment tab of the Incident Register window covers which area of the company the incident occurred in and what the working conditions were leading up to the incident.

Location

Primary Incident Location  Specify the working environment where the incident took place. Two list boxes are available: the first lets you specify the general type of location, while the second box contains sub-categories that further specify the location in the first box.

If none of the categories are appropriate, select the "Unspecified place" option.

Actual location or address  Give the full street address of the premises in question.

Did the incident happen at the work site?  Select "Yes" or "No" from the list box.

Which risk area?  Enter the relevant Risk Area code or click the button to select one from a list. This helps to establish working conditions and practices leading up to the incident.

Is a Hazard Involved?

Tick the check box in this heading if the incident involved one of the Hazards defined in the system.

Which hazard is involved?  Choose from the list of hazards you created in the Hazard Register.

Treatment

Is there a claim?  Tick this box if you intend to enter a Claim for this incident.

Treatment of injury  Specify how the injury was treated. Choose from:

  • Nil
  • First Aid
  • Doctor
  • Hospitalised
