Role Form
This form contains related person role information. It displays the currently selected record code and description, what the related person is in relation to the record based on where this form was opened, the role of the person, date from and to the person has held the role and a description of their role. The related person can be a committee member or medical person.
Options Reference
Each option and field on the form is described below. Mandatory options and fields are highlighted in red.
Role
Enter or select the role type from those available in the dropdown list. You can add other roles.
Status (only available for committee member)
Select the status for the related person. If a related person is inactive, emails are not sent to them. You can add other statuses.
From (only available for committee member)
Enter the date from which the role applies to the related person for the selected item.
To (only available for committee member)
Enter the date to which the role applies to the related person for the selected item.
Description
Enter text to capture the description of the role for the related person.
