Incident Register - Investigation
The Investigation tab of the Incident Register window covers the potential repercussions of the accident on company employees and property.
Is there an investigation?
Tick the check box in this heading if an incident investigation has been carried out. Ticking this box enables the questions below.
How serious could the incident have been? Indicate how serious the incident could have been: Very Serious, Serious or Minor.
What is the chance of it happening again? Indicate how frequently the incident is likely to re-occur: Frequent, Occasional or Rare.
Damage
Is there damage? Tick this box if company property was damaged during the incident. Ticking this box enables the questions below.
What sort of damage was inflicted? If any property was damaged, describe in one or two sentences what physically happened to the property.
What property was damaged (if any)? List any building, equipment, parts, tools or machinery.
What object or substance caused this damage? If any property was damaged, name the cause of the damage, e.g. fire, smoke, water, land movement, electricity, chemical reaction, collision, falling.
