Claim Maintenance - Details

Incident Code  This is the unique code assigned to the incident that the claim relates to.

Employee  This is the employee who had the accident in question.

Details

Type  Specify the type of incident. Choose from:

  • Accident - In which the employee was significantly injured, or property was significantly damaged.
  • First Aid - In which the employee was not significantly injured, but for which first aid had to be rendered.
  • Incident - In which the employee was not significantly injured.
  • Near Miss - An event in which an employee narrowly avoided an injury.

The recommended type for a claim is "Accident".

Date  Enter the date on which the incident occurred.

Time  Enter the time at which the incident occurred.

Shift  Select the shift that the incident occurred during: Day, Afternoon or Night.

Where did the accident occur?  Select from the following list:

  • New Zealand
  • Australia
  • New Zealand territorial waters
  • Australian territorial waters
  • International waters
  • Other country

In which other country?  If you select "Other country" for the previous question, enter the name of the country into this field.

Actual location or address where the accident occurred:  Enter the street address, or if there is no address, directions to the accident location.

Did the event happen at the work site?  Select "Yes" or "No".

Which department or area?  Enter the physical area of the business premises that the incident occurred in.

The accident happened:  Specify where the employee was in relation to the workplace at the time of the incident. If none of the available options apply, select "Other - please specify".

Please specify:  If you selected "Other - please specify" for the previous question, describe in your own words your location relative to your workplace at the time of the accident. Note key points such as In Transit, Entering, Exiting, etc.
